Waitlist Intake Form

Hello, and thank you so much for your patience in standing by on the waitlist since we last spoke! I’m looking forward to meeting you and taking on the honor of taking care of your pet for you ❤︎

To schedule your drop off, please refer to the scheduling link that was sent to you through my last message to you. Please fill out this form only in the same sitting that you schedule your appointment for drop off. You have from now until February 15th to submit your intake form and schedule a drop off. After then, your spot is forfeited and made available for another recipient. (You can book any date, the form just needs to be submitted by February 15th).

After filling out this intake form and scheduling your drop off, I will contact you with a confirmation of your appointment, and further details/deposit amount required.
