Waitlist Intake Form

Hello, and thank you so much for your patience in standing by on the waitlist since we last spoke! I’m looking forward to meeting you and taking on the honor of taking care of your pet for you ❤︎

Please fill out this form only when you are ready to schedule a drop off for your pet. You have from now until February 5th to submit your intake form. After then, your spot is forfeited and made available for another recipient. (You can book any date, the form just needs to be submitted by February 5th).

Intake drop offs are scheduled on Sundays, Mondays, and Thursdays between 10am-7pm. I will be unavailable 1/29 - 2/2.

After submission, I will contact you with a confirmation of your appointment, and further details/deposit amount required for your drop off. If you have any other questions before filling out, please refer to the FAQ, or reach out anytime, I’m more than happy to answer any questions.
